Residential Monitor
Under the direction of the Program Manager, the Residential Monitor is responsible for monitoring the functional operation of the residential program and facility under the supervision of the Program Manager to ensure safety and regulatory compliance. Monitors facilitates supportive interaction with program participants and team members, assessment of participant’s needs, and performance of all work duties as required.
Drivers
Under the supervision of the Program Manager, Drivers are responsible for transporting meals, participants, good and service to and from sites, social supports and benefits related appointments. This includes transporting goods and services and/or clients between housing units and program sites in addition to breaking down and storing donated goods and service. Participating in outreach activities.
LVNs
Under the direction of the Clinical Director, the Licensed Vocational Nurse will provide nursing care to all HALCDC clients, within the LVN scope of practice in a community homeless shelter setting. LVN will be responsible for conducting vital sign observations, administering ordered medications and treatments, and reinforcing the physicians’ instructions to the patients.
Intake Specialist
Under the direction of the Program Manager, the Intake Specialist serves as the first point of contact with guest and conducts eligibility screenings and processes that include: orienting clients on agency services and policies, copying and collecting required compliance documents, completing required forms, and performing data entry in CLARITY and other related clerical activities.
Case Manager
Under the direction of the Program Manager, the case manager provides ongoing expertise and support through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of this position is to enhance the quality of client management and satisfaction, and to promote continuity of care and effectiveness through case management, utilization review and ongoing interaction with client. The case manager is responsible for monitoring the functional operation of the residential program and facility under the supervision of the Program Manager to ensure safety and regulatory compliance.
Mental Health Therapist
Under the supervision of the Clinical Director, the Mental Health Therapist (MHT) will perform professional clinical services in connection with treatment of mentally ill and emotionally disturbed consumers who are experiencing homelessness. MHT will be responsible for providing individual, family, and/or group therapy as well as participating in multi-disciplinary team meetings. MHT will assist consumers and their significant relational figures in understanding and developing solutions to problems that leads to and results from emotional and mental disturbances. Incumbents of this position will collaborate with housing supportive staff and collaborative partners. MHT will engage in both individual and group supervision. MHT will provide bio psychosocial assessments and formulate effective treatment plans that correlate with differential diagnosis. Mental Health Therapist will comply with all ethical/legal mandates for reporting; provide psychotherapeutic methods in assessment, implementing treatment plans in order to develop a corresponding progress for various mental disturbances. Mental Health Therapist must be able to ordinate the workday responsibility in order to services clients, attend meeting and participate in trainings.
Program Manager
Under the direction of the Housing Supervisor, the Program Manager serves as the Shift Lead including but not limited to supervising intake specialists, residential monitor and case manager; serving as the first point of contact with guest; and conducting eligibility screenings and processes. This includes orienting clients on agency services and policies, copying and collecting required compliance documents, completing required forms, and performing data entry in HMIS/Clarity and other related clerical and case management activities.
