Summary
Under the direction of the Clinical Director, the Licensed Vocational Nurse will provide nursing care to all HALCDC clients, within the LVN scope of practice in a community homeless shelter setting. LVN will be responsible for conducting vital sign observations, administering ordered medications and treatments, and reinforcing the physicians’ instructions to the patients.
Essential Functions
- Conduct initial health interview /intake and client health needs assessment
- Gathers data relevant to the patient’s individual needs and age group
- Elicits important information concerning the patient’s progress
- Gathers data regarding patient needs while providing direct patient care
- Assumes responsibility and accountability for designated patients
- Provides nursing care for all patients within scope of LVN practice
- Identifies self as nurse responsible for patient care to patients, family and physicians
- Documents nursing observations according to hospital charting standards
- Administers medications by injection, orally, and other pre-determined methods as determined by the Medical Department and client treatment plan
- Checks prescribed dosages against specified limits
- Reports symptoms, reactions and progress of patient to the clinical supervisor and/or charge nurse and appropriate action has been taken
- Pursues proper chain of command to attain a satisfactory solution
- Initiates changes to improve patient care; discusses changes with doctor when appropriate
- Provides IV Therapy according to nursing policy and procedure that is within LVN scope of practice
- Measures and records vital signs; records bed care procedures, treatments, indications and nourishment provided and output
- Recognizes the need for emergency care; initiates life-saving action in emergencies and alerts the proper emergency medical professionals including nurse-in-charge or physician
- Provides patient and family education
- Responsible for knowledge of legal aspects of nursing
- Attends fire and safety classes annually and maintains a current CPR (BLS) card
- S/he will assist in trainings and provide support to necessary staff, providers, and other project partners participating in project
- The LVN will participate in all mandatory program and division meetings and trainings, as assigned by his/her supervisor and division director
- Participates in treatment planning and multi-disciplinary team meetings
- Prepare project reports in accordance with funding requirements
- S/he will assist in trainings and provide support to necessary staff, providers, and other project partners participating in project
- Maintain appropriate boundaries, and adhere to HAL’s Code of Ethics and HAL’s Core Values
- Represent the Agency in a professional manner at meetings and community events
- Regular attendance required
- Other duties as needed
Secondary Functions
Perform other duties as assigned by the Clinical Director, Director of Housing Services and/ or Executive Director. Answer phones and route incoming calls; serve as back-up to other Case Managers in his/her absence. Administering Nose and/or Mouth Covid-19 testing.
Minimum Qualifications – Knowledge, Skills and Abilities Required
Must be currently registered with California Board of Vocational Nursing (active and unrestricted) and be certified to start IV’s and administer medications. Current certification issued by the American Heart Association’s Basic Life Support (BLS) for Healthcare Providers (CPR & AED) Program. Previous experience as an LVN in an acute general hospital or in homeless shelter is desirable. Must present a professional demeanor. Must have good interpersonal skills. Must maintain a calm and reasonable attitude during crisis and conflict. Must demonstrate initiative and good judgment. Must respond appropriately to routine and emergency situations. Must be able to set and revise priorities based on changes in the situation.
Working knowledge of Microsoft Word, Excel, and other database programs. Accurate and concise documentation. Neat and professional appearance. Attention to detail and punctuality required. Ability to communicate effectively, both written and orally.
Verification of Employment Eligibility and Background Clearance. TB test required (Not more than (3) months prior to or (7) days after Date of Hire, and renewed annually thereafter), CPR and First Aid Certification required within 30 days of employment with company and valid Driver’ s License and auto insurance required. Reliable transportation is required. Ability to communicate effectively, both written and orally.
Supervisory Responsibilities
This position oversees Residential Monitors, Case Managers, Interns and Security.
Environmental Conditions (Working Conditions)
This position is responsible to work in “client friendly” environments and is required to be located at a homeless shelter as part of their duties. Local automobile travel is required. There is some responsibility to work in noisy environments where adults are present. Will be working in an environment with asymptomatic COVID-19 participants who are under quarantine. PPE Gear is provided by the HALCDC and must be worn at all times.
Physical Requirements
The LVN typically spends time sitting, standing, typing, thinking, writing, walking, driving, lifting (max. 25 lbs), listening, speaking.
Mental Requirements
This position will require the individual to be able to handle any/all of the following: constant distractions, interruptions and uncontrollable changes in priorities/ work schedules. Must be able to process information, think and conceptualize.
